Thank you for considering our services. Most GPs or other providers will send a referral by Fax or email. This page offers a downloadable referral form which can be faxed or emailed to my practice.

I am always happy to discuss a patient with you if you need some advice. The best way to do this is to call my rooms on and they will put your call though to me or text me your number and a message to call ASAP. I will return the call as soon as I am able.
